If you had a preceptor that constantly violates HIPAA what would you do? Is there a way to report him anonymously?

Graduate, pass boards and do not violate HIPAA when you are practicing.

"Deal with" are pretty strong words for anyone, especially in a student/teacher relationship.
If you are having a difference of opinion as to what constitutes these violations, have some courage of your convictions and discuss your concerns with this person who I will presume is doing you a favor by teaching you for gratis.
That is unless you feel honesty is for suckers and Machiavellian tactics are a safer means to achieve your ends.
Remember character does count. Not with many these days, but it does count.
As in leaving lab results including STD results on answering machines even if the name does not match or texting patient information etc
