How to correctly document changes on LTC MARS

Hi All,

Hope someone out there can help me with this problem. I work in a facility that uses the month long, LTC type of MAR's. I'm looking for a site, book, article, etc. that outline the correct way to make changes on this type of MAR. There are several nurses that have come from all over the county working here, and everyone is thinking that their way is the correct way (as do I :D) and I'm looking for something that will cut through the confusion and put us all on the right tract.

Does your facility have a policy on how to do this? If they do, following it would be the easiest way.

No, that's part of the reason I'm trying to find a reliable source for this information. Both the DON and the administrator have less COMBINED experiance than I do, and I'm tired of :banghead: with those two.

I haven't found a website yet. However, the way that we document changes is highlight the whole space through the end of the month, then write D/C, date and initial. For Coumadin changes we will write "Hold' in the spaces if only holding 1 or 2 days. If longer we get a D/C order, note it in the chart and then resume with a new order when labs indicate per MD. That prevents med errors. I hope this helps :)

we so the same. Yellow out the whole space. If you don't have a highlighter at that momemt...we put a line or make sure that the entire month is crossed off and DC is writtne in that space.

How not to do it.....scratch off or x off the change such as q day or bid and just keep it in the same spot writing over the change.

This came up recently where I work with the crossing off and writing in of new times 3/4/ of the way through the month.

I agree with other posters, as to how it should be done.

If there is not a policy try calling the pharmacy consultant for your building, they could tell you the correct way and probably send you documentation for your facility.:wink2::wink2::wink2:
