Hello everyone! I would like to ask for your help ,

So i answered kaplan's question from qtrainer and qbank.. its about sickle cell, accdg to kaplan sickle cell crisis management first priority is to give IV for rehydration ..so i answered it and its correct,.then i am in lacharity's book the PDA under hematology chapter , i answered same question, since kaplan said its priority so I answered IV to rehydrate the pt. but its wrong ..lacharity says to give 100% O2 since oxygenation of sickling cells help alleviate sickle cell crisis...i am torn between who to follow when i sit for the real exam ..pls help me :(

i think the treatment depend on the crisis if he has symptoms like cough,chest pain,shortness of breath which is called acutes chest syndrome than the treatment will be oxygen if Painful Vaso-Occlusive Crisis than hydration and painkiller tylenol..

Thank you for your response.. I reread the kaplan's rationale and says "IVF for all sickle cell crisis" as oppose to lacharity

Funny! I was just having this conversation with a friend today...Bc ATI said that having the patient REST was priority over IV fluids!

Rest over fluids or O2??? What was the rationale?

It said - they have a high cellular o2 need so RN should reduce metabolic demand by making them rest

I wanted to bang my head on the computer when I read it!!!

I dont have ATI but its my first time to hear rest over fluids and o2..

i am so confused with who to follow when i get this question in the real exam.. :(

what was the question? maybe it differs with how the question is on ATI.. But mine have same question on kaplan and lacharity so im really confused..

I would not listen to ATI on this one! My friend and I were baffled!!! I wish I could get back into the assessment to see exactly the wording. What my friend and I concluded was - IV fluids first (Bc to stop the crisis, they need to be hydrated so the sickled blood stops sticking to the vessels) then strap O2 on them, then pain management.

This is why we figured it in that order, more O2 won't matter if the circulating blood isn't able to utilize it because it's all jammed up.

And yes, the patient should be resting to decrease the O2 demand but I just don't see that being #1.

In response to the ATI question, I just had my two day NCLEX review course with a ATI representative and we had a question just like this except the question just stated they had sickle cell. It did not state they were in sickle cell crisis. So the answer was to rest. The representative stated if the question said they were in crisis then the answer would have been different, such as O2 or IV fluids, But at this time rest was the best answer.

What about Kaplan's ABC's? Breathing (02), before Circulation? (fluids)

I just dont know because the situation in kaplan and lacharity are the same they are both in sickle cell crisis so when i answered my kaplan qtrainer 4 i answered IV because kaplan says so and it was correct but as for lacharity its supposed to be O2.. Can i post the exact question here and we could help each other in figuring it out?..i really would like to know which one to prioritize because if it happens me or anyone of us got a question like this in the real exam..we can answer it right and might help us in passing the actual exam..
