Help with nursing care plan

Need help with my care plan.!

My p/t has edema on her LLE. She had hip replacement surgery.

I was going to use fluid volume excess r/t decreased mobility AEB edema of the LLE.

My student nursing care plan calls for three goals: long term goal, and two short term goals with three interventions for each short term goal. HELP!

What is your question - what kind of help are you looking for?

Sorry, wasn't specific enough. I need help with another short term goal and interventions.

One of my short term goals is "Edema will not increase in circumference while in my care, 6/25/2015, 7-3". Interventions for this are:

Assess p/t for edema and measure the circumference of the LLE.

Educate the p/t on the importance of mobility.

Encourage passive/active range of motion every 2-4 hours.

My long term goal is "Excess fluid volume will be eliminated." Is that correct?

Also, is my diagnosis an appropriate diagnoses: Fluid volume, excess, R/t decreased mobility, AEB Edema of the LLE.

It seems to me like you're on the right track, but was starting to doubt yourself. I don't do formal care plans on a regular basis but this looks okay to me. :)

You're getting there. Remember that you need observable interventions. You forgot the time frame for the assess edema intervention. When will you do it?

Do you have a care plan book? I recommend this one. It has everything you need in it.

You have to decide how frequently the pt needs the intervention ( dependent on how sick they are)
