i need some help
i am writing a careplan about diarrhea and i got to implementation but cant think of anything reasonable.
my nursing dx is ( diarrhea R/T adverse efferct of antibitic AEB going to the bathroom 10 times per day.)
my goals are 1 Patient passes soft, formed stool no more than three times per day.
2 pt will have fluid balance by feb 25th

provide or encourage high fiber foods

monitor therapeutic levels of antibiotics, if ordered (such as with Vancomycin)
forgot most obvious - provide replacement fluids

for the life of me, i don't understand why students come up with goals and have no idea of how they are going to achieve them. your goals should be what you expect will happen after your nursing interventions have been performed. that means you already have an idea of what your nursing interventions are going to be. look in your nursing books and see what they say you can do for someone with diarrhea. if that doesn't give you ideas for nursing interventions look at these two websites:
