Can I just ask how many were asked to fill out a health form for the nursing school and how involved it was?
Did it ask for previous surgeries, medicine you are currently on, a copy of your insurance card, etc.
Also, did your doctor have to sign off on it?

Yup! Everything you just said, it was asked. It was pretty much just a physical form and the MD had to sign off on it.
Our school also wanted a copy of our insurance card, front and back along w/ our immunization record, 2 step TB test results, CPR/BLS card and I think thats about it medical wise.

We have a form that me and my doc signed that basically said I was capable of the lifting and general physical requirements of nursing, including being exposed to various diseases, etc. There weren't any specifics about past medical hx. That is attaches to the immunization verification papers.

We had one. It had a yes/no kind of medical history section, conditions we are being treated for, and other basic things you would find on a physical form.
We also had to have all our shots, CPR certification, TB test.
There was also a section for the MD to verify that we are mentally and physically capable of being in nursing school.

I'm not yet in the nursing program. I'm a medical assistant in a PCP office, so I've seen plenty of NS forms. Each school's form is a little different, but most seem to be simple yes or no questions for the doc to check off. Its really quick and easy. I actually think most pts would have the form back the same day if we didn't have to wait for the titer results (usually Hep B, varicella, MMR and polio.)

Barely asked anything at all. A few yes/no questions and a place for the MD to say she thinks I am fit enough.
There is NO chance I am sharing a detailed medical history with my school. They have never been able to tell any of us who is allowed access to it. They just say it is "need to know". All my school "needs to know: is that I am fit for the job. The end.

I had to fill out a health form and show proof of insurance but I didn't have to have a doctor sign off on it. We just had to make an appointment with the school nurse and she quickly went over the info on the health form. It took about 5 minutes.

be careful about witholding information. i have a mental illness that was disclosed, and the director of the program did not even bat an eye. she saw i was stable on medication, that i maintain constant supervision from my physician, and that i am healthy to practice.
we were informed that witholding information was grounds for expulsion from the program, no questions asked. if we missed school for an undisclosed illness, we could be let go for non-disclosure. it is better to be honest.
