Updating Care Plans

I work in an acute care hospital. I would like to know is there is a mandate out there that tells us how often we are required to update care plans. We currently update/re-evaluate acute care plans every 3 days and update/re-evaluate skilled/swing bed care plas weekly. I think it should be more often, but need facts to show this. Any help is greatly appreciated.

I thought Joint Commission wanted them updated every 24 hours in an acute care setting. That is our procedure. You expect things to change faster in an acute care facility than in LTC. If goals are not met then should pt be discharged???

Wow! Been a long time since I did Med-surg nursing and they were updated every month :)

We update at least every 24 hours for Joint Commission.

We update each shift, so twice a day.

Updated every 24 hours by an RN and prn at my facility

I have spent several hours now searching through the Joint Commission website and I still can not find documentation for care plan updates every 24 hours. Anyone have any search type suggestions?
