Tools for assessing dementia levels?

Hi everyone,

I recently started a new job in an Assisted Living complex. My boss has asked that next week I do an assessment on a potential client and see if they are appropriate for assisted living. Since there wasn't really a full time nurse before I started a week ago, there are not really any protocols, paperwork or tools that I can access. I was wondering if anyone had some suggestions on papers that I could fill out to help me assess this potential client?? I am planning on doing a mini-mental, but anything else you could suggest would be great!! I don't have any information on her except that she may have some mild dementia, her family is expressing care giver burn out, and our provincial health system has not been helpful to them.

I would also ask questions on what her functionality is like i.e walking, risk of falls, had any falls recently, what aides are required, what assistance is needed, can she feed herself, dress herself. What did she do when she was younger/fitter. Any hobbies or interests/family

Here are a number of them. I would encourage you to read them, pick one or develop your own with the relevant components, and use it consistently. I have been involved in at least one case where an admission to assisted living was inappropriate and there was mention, but no copy, of an assessment tool used at initial eval by the RN. The resident turned out not to have been remotely safe for assisted living-- couldn't do basic ADLs with minimal assist, no safety awareness, very unsteady on her feet so could not be left unobserved, e.g., in her own unit, etc. -- she died a few days after admission after a fall.
