Report/flow sheets

Does anyone have report sheets or flowsheets that they find useful for work? I have the VS flowsheet they use at work, but I'd like to have one to keep for myself...one that is for each system, has space to write stuff etc. I wish I knew how to make one up myself, not really computer savy...Would greatly appreciate it. I have a sheet that I use everyday at work. Everyone on my unit uses the "brain" that they find most useful to them. One day I was out of mine and borrowed one from another and couldn't wait to print out more copies of my own. Anyway, mine is based on 3 patients, because that is how many I will have in one day (unless admitting or discharging, which is not a daily thing on my unit). One side of my sheet has pt information/systems based, and the other side has the hours of the day for me to write my meds down. The systems side is printed on one side of the sheet of paper, and the hours of the day is printed on the other side of the paper so its a double sided single sheet of paper with writing on both sides. I am attaching it. I fold the paper in half and then in thirds along the columns so it fits my paper. Not sure if this is what you meant, but may give you an idea for organizing your day.
