Has anyone ever heard of a nurse being terminated for gross misconduct after accidentally omitting an entry when transcribing MARs from one month to the next?

Oops! I forgot a couple of things: the month/year and the route.

This!! And clearly there are systems issues here. Since the MARs are sent from the pharmacy, how in hades is the nurse 100% responsible for a med error reaching a pt? Where is the PharmD's responsibility?
This is something I've never understood. Pharmacists make a ton of money, at least 4 times what I make, yet they are rarely held responsible for med errors. Only the nurse.

Nope, spent many years transcribing MARS before they were computerized. And if any error was made, it was the nurse's fault, period.

Gak that would never fly here.
If a patient needs a new chart, doctor has to do it. Although it is the nurses responsibility to chase it up with the dr
I don't deny I made an error but "gross misconduct "?

my heavens, who is still using paper?
My private duty agency. I have to transcribe each week. Actually, I double check the printed MAR that the nurse supervisor of the case sends to the homes after SHE transcribes each week.
