Making my Health Care Proxy

I need a health care proxy. I'm tired of seeing helpless folks at work without one getting stuck with vents and g-tubes. All of this being said, I'm confused about how to word one. "No artificial nutrition" comes to mind I guess but does anyone know some key phrases that I can put on my health care proxy that shows that I want nothing to do with a vent, g-tube, cpr, IV, or anything else that might extend my vegetative life further. Good pain meds only! I'd also like to put in there that I want my organs donated entirely and my body donated to science. I need to toss the organ/body donation thing in because my husband doesn't quite agree with me there and although I'm sure he would follow what I would want, I want to make darn sure. ;)

You should use a lawyer or program to write it for you. You are less likely to forget something this way.

I personaly used the Family Lawyer disk to make mine. I'm very happy with it.

I agree. If you are not able to have an attorney prepare your documents I strongly suggest you look for a legal blank specific to your state. I have seeen them sold at Office Max for about $4. Also, you might try typing {your state} power of attorney forms (or health care proxy forms} into a search engine. It is also possible that your states DSHS or Dept. of Aging has a free blank.
