Do any of you have a Parent Questionnaire for students who have a Food Intolerance (not Allergy) that you would be willing to share?

Nope. Don't open that can of worms. Parents will list it under an allergy or the"other" category if they're truly concerned.

No. Non-sanctioned health questionnaires are frowned upon by my supervisor for the liability we open ourselves up to. In my school, we already have enough questionable "allergies" and "intolerances" that doctors will sign off on. I've been at this only a year and I can't even count the medically documented dairy allergies we abide by, but then the parent shows up angry because the kid was denied pizza at lunch.

We have a line for "food restrictions" on our health form. It specifically says "Food restrictions (i.e. vegan, kosher, etc.):______________________________" this is where most parents will indicate any food or dietary restriction. The parent is free to write "lactose intolerant, no milk or cheese" or "gluten intolerant, avoid gluten" on the short line. If I have questions I'll follow up with the parent.

Yeah - don't go there. Only thing we have is a couple forms from the nutrition department for parents and/or physician to complete. One is a form for the physician to complete if it is a true intolerance/allergy and there is a another "food preference" form for issues such as reporting a "vegan" "No pork" etc etc
