Finger print clearance card

ok now, this whole process is driving me crazyyyy...

i got my cna back in june. i submitted my application to dps for finger print clearance card in order to get my cna license from azbon....i never got the card, however, i received my cna license. now when i want to apply for lpn fast track, i need my finger print card, which i don't have. i called dps they said they have no records of me. i called azbon; they said they don't even get that card back from fbi.

do you know anything about this? for all cnas, did you get you card?

now i have to submit another application to dps and pay 47$ for the card, and what is worse, i have to wait a month to get that. application process for lpn starts in september.

Do you have a copy of your fingerprint clearance card application?

I could swear I remember that the form itself was numbered in the upper right hand corner... maybe that would help them locate your info. :confused:

Another option might be if you paid by money order(hopefully you have the receipt) which you could obtain a copy of the item that was used to pay in order to prove to DPS that it was already completed.

I think I have copies of everything. I will look when I get home. So, you think that I should be able to get my card?

I would think that as long as you have a copy of your application AND proof that the money order used to pay for it was cashed by DPS they should be able to do something for you.

If that is the case, it sounds to me like someone at DPS dropped the ball.

I have had to request copies of money orders before - - it can take a while depending on who you purchased it through. Bank money orders tend to be a bit faster to receive... money orders purchased at grocery & convenience stores might take a while. The receipt is key.

So, does anybody know for sure, if you get your CNA license, are you supposed to get your finger print card as well?

This was confusing me too, but I think I know what happened? Maybe.

I went to RSAA and I wrongly assumed that when they said they do your fingerprints and the CPR class that it was what I needed for nursing school.

WRONG.

You must submit one set of fingerprints to AZBON and ANOTHER SET of fingerprints to DPS with an application and a check for $47.

This has nothing to do with fingerprints, but not only do you get a CPR cert at CNA school, but it's not the one you need for nursing school so you then have to take a completely separate class to get into NS.

Ugh. Confusing!

Anyhow, is that what happened, that you never sent in a 2nd set of prints to DPS? If you did, and you filled out the application and already paid the $47 I have no idea! That sucks either way. I'm in a somewhat similar predicament.....waiting on my clearance card and my CNA license number for January placement (application must be in by October)

When you look on the AZBON site it should say that you have your figerprints on file. Like Fingerprint- YES or something like that. As you received your cetification. At CAC they only need proof that you are certified in arizona because you can only be if the fingerprints are on file and ok. At least thats what I heard. You don't get an actual card maybe?

Or you have to send in two sets??? I have not heard of this

You are required to complete and submit ONE fingerprint card with your application that the BON has sent to you. The board will submit it to DPS and the FBI for processing.

IE if you sent in the card the AZBON sent to you then your CNA is official and because its NEW you should not have to submit an additional figerprint card for the program as this CNA should meet the qualification. Ask your program first before going crazy.

RSAA told me we HAVE to send in 2 cards. One to AZBON, and one to DPS. You could call DPS and verify, but this is what I was told and this is what I did.

I just called RSAA and they confirmed to me that you DO have to submit 2 fingerprint cards like I previously stated. The set you sent to AZBON is to get your background check done and the one sent to DPS is for the fingerprint clearance card.
