Say, hypothetically you have all the money in the world at your disposal.:chuckle

Patient is 90 yr old NIDDM patient with dementia and constant oozing of stool. Her status is palliative, and decision has been made to do no aggressive forms of treatment. Diet is tollerated very well, but is limited because of NIDDM status, swallowing difficulties and various other feeding complications. Has indwelling cathether and is bedbound with multiple contractures of the legs. Stage IV ulcer to coccyx has been present since 1999 (!). Multiple products have been used over the years, multiple clinical nurse speciallists and infection control specialist have been consulted. Pt had been on VAC therapy for 6 mo, but this lead to a deepening of the wound bed, and was d/c'd in 2001.

How do you propose to treat this client?:)

Wound is granulating. Imagine! This is a wound that has been there since 1999. The most lo-tech solution is working. Did I ever mention that in my last job, we healed a stage IV ulcer using only guaze, hydrogel and hypafix.

Have you tried regranex? It contains human growth factor...usually used for diabetic pts. Extremely expensive. I think there is a website regranex.com

Nutrition is definitely key....is she on multivits? Protein, Vit c, zinc.

Yes she's on a multivite OD with extra minerals like Zinc. I also think the the protein supplement is doing her a lot of good.

I have heard of regranex. It has been used in the subacute section of my facility and has worked well. We do not have that option, because the unit cannot absorb the cost and the patient is what you would call a ward of the state (she is under the public trusteeship). Canadian Healthcare has its positives and negatives.

adrienurse: how is the hydrogel working? There is a very good protein supplement made by Novartis called Arginaid. I have tried it and find it is accepted (by the resident)better than Promod or other products used. I am not sure of the cost. http://www.Novartis.com

Quick Thoughts.

Regranex is bioengineered platelet derived growth factor, (PDGF). Good stuff when accompinied by excellent wound care. I use it on small lower extremity wounds, like heel wounds that were unstable, debrided to calcaneous and then I might use Regranex to help fill the wound with granulation asap.

Ortho-Mcneil started clinical trials with Regranex on pressure ulcers, but reportedly the results were less then stellar and research targeting FDA indications for pressure ulcers were stopped. As suggested, Regranex runs, best price ~ $425.00/15 gram tube. If a wound is big enough, could possibly go through a tube in just a few dressings.

Arginaid is a suppliment with L-arginine, an amino acid that is a precursor to nitric oxide, a potent vasodilator that may increase circulation at the wound site. L-arginine is one of the few nutritional suppliments that has received favorable results in the medical research. However, L-arginine by itself is not a protein and I believe the prior versions of Arginaid offered very little complete protein. Best to suppliment the individual suffering with protein-calorie malnutriton with easily digested protein at a general rate of 1.5 to 2.0 grams of protein per kilogram of body weight per day. Might consider combining Arginaid with other high protein sources so long as renal function isn't impaired. The original Arginaid cost was about $1.75 - $2.25 a packet and recommendations was 2 packets daily delivering ~ 8 gms of L-arginine daily, but again, very little protein.

We just started using Arginaid at our facility. the residents like it better than the Med pass supplement that we were using.

we have noticed that alot of the ones with chronic wounds are starting to heal faster!
