Exam Cram is a good book with lots of useful information and strategies. When I started to use the tests (at least in the CD) I became suspicious. The questions are very tricky. I understand this is also the nature of the NCLEX but I think EC goes a little too far and run right into not being accurate. For example
1) Which of the following is common with clients with duodenal ulcers?
a) Epigastric pain after meals
b)Frequent bouts of diarrhea
c)Presence of blood in the stool
d)Vomiting shortly after eating
All the books I looked said that a,b,d are the common symptoms with duodenal ulcer.
Correct answer: C. Blood in the stool is a complication and a less frequent event. The most common is pain after meals.
2)On a home visit, the nurse finds four children alone. The youngest of the children has bruises on the face and back, and circular burns on the inner aspects of the forearm. The nurse should:
a) Transport the child to the emergency room
b)Contact child welfare services
c)Carry the child to an abuse shelter
d)Stay with the children until an adult arrives
Correct is B
But wait, aren't we suppose to stay with the patient?, aren't we suppose to take care the patient physical needs first, and after that is resolved contact an agency?
No, there is something strange about these questions...I will stay away from them, what do you think?

i also used EC and that question no. 2 hit me. lol i was really confused about the rationale... and been thinking... since the children were suspected "child abuse" nurse should contact welfare for their security first... not so sure bout the idea but is that make sense??

I did have issues with the duodenal ulcer question but I know that they say no matter what if you suspect abuse in elderly or children you should contact the appropriate agency then stay and wait for them to arrive. First and foremost is there safety. Dunno just my thoughts. But overall I really liked the exam cram. There were some issues but overall I thought it was good.

c)Presence of blood in the stool - duodenal ulcer
d)Vomiting shortly after eating - Pyloric stenosis
a) Transport the child to the emergency room- Not in life threatening situation
b)Contact child welfare services- If a nurse suspect any abuse, requirement is to notify the welfare service also known as CPS
