Who uses something like this? We hand them out on the first day every year and they basically cover emergency contact information, brief medical history, and consent for us to provide basic first aid etc. Then the back is a template for 'charting' however everything is computerized and nothing is ever documented on them (I feel it is a waste of space because if the school computer system would go down I would use another template I have printed out until I could get in the computer to document everything)
Anyway I am looking to revamp ours (this is what was used when I started here)--I feel like its too wordy, parents fly through it and do not even pay attention to what their filling out. I also would like to add a section regarding other siblings in the district (name/grade) since a lot of them have different last names. Those of you who use something like this what does yours look like? I'd like to avoid using the back of the form because parents NEVER check the back of forms and I always have to send stuff back home to be completed

This is ours. They're kept by our front desk, but I give them a quick look in the beginning of every year in case something is mentioned on there & not on any medical forms. All info is put onto our computer system by our receptionist.

Ours are all electronic. Parents are asked to fill them out by the first day of school. If not done, they get ones mailed to them 2-3 times before district level Administrators discuss it being a policy violation.

Mine is double sided. I use as my run sheet for EMS as well. I copy it and write any other details they need to know on the top (vitals, etc).
(Of course, tracking down an updated one each year is UGH.)

Thank you for the ideas!! I actually created a new form kind of combining my original card with some ideas from your cards! I really appreciate your help!
