when you recieve packet 4 and you make your embassy medical appointment, i assume you have to fill in a form about your medical history?
does this need to be confirmed or countersigned by your gp? or do you just fill in the questionnaire yourself?
im just wondering as obviously i will need to make a gp appointment for this and there will probably be a fee! (again)

yeah i used to take it for migranes but since i stopped taking the contraceptive pill i dont get migranes anymore thankfully
when i injured my back recently i was advised to take it then but i didnt want to risk it so the doctor gave me diclofenac which has done the trick.
thanks for clearing that up apriljan i expect the form will be 300 pages long like evrything else lol
No your GP doesn't have to fill in anything. You have to sign it to say you have declared your medical history truthfully. Mainly a yes/no form, but obviously if you have ticked yes they require further info. Pretty straight forward really.
