Hi all,
I am starting a tread for all the students that will be applying for spring 2015 ADN program at El Centro. Please join and give any info that you might have. I want to be successful as well as everyone else trying to apply.
I want to say thanks to everyone on this blog. If i would not have checked yesterday i would not have known the change until it was to late. Im headed to the Dallas County health Dept for hep A shot in a moment it costs $50. I wonder what will happen with applicants that already submitted ther applications before the sentry md change? Good Luck everyone were almost there!!!!
I just got my Hep A shot yesterday and submitted my SentryMD paperwork last night via e-mail. It sounds like we no longer have to submit the immunization from with our nursing packet (in the 9"X12" envelope)......right?So if that's true, our nursing application will include:
-The completed nursing application form
-signed statement of student's responsibility
-Official Hesi A2 test scores
-and any course substitution forms that you may have
For me, that's only three pieces of paper. Does that sound right?
That sounds about right. I guess we turn in our CPR verification along with the physical exam form and the proof of our insurance? We have no Nursing blue card yet so thats out of the picture for now I assume.
Turned in my application today and was suprised at how quick and easy it all was. Just a little mail flap on the side of the main door. I actually felt quite dumb for paying the 5$ for parking. I had a friend with me she could of stayed in the car.
I turned in 3 pages to El centro in my 9x12 envelope labeled to El Centro and I still put my name and address on the front just in case. I turned in the Hesi report, the student responsibilty and the application sheet.
For sentry md I went to the website they give and I paid the 15$ through paypal then I emailed them. I scanned my physical exam (4 pages), cpr card front and back (2 pages) and saved a pdf of my insurance card just in case(1pg since I went to their webpage and its front and back there). So I made a compressed folder with 7 images in it. I made a compressed folder because I was having issues attaching 7 items to my yahoo email.
Well I hope this helps someone I am finally done with these classes until fall. I will be awaiting my denial letter in October because of my second HepB shot. That is fine I will now know what I need for next time and I will retake the Hesi A2.
I go on my much needed vacation tomorrow to SPI. Good Luck to all the Hopefuls. Remember if you get in this stress is nothing! If you dont get in (like me) just keep trying for your DREAM!
@casandrakcvegas, don't lose hope, just like briana stated! I'm surprise to see you paid $15. I actually went on their site this money, and I paid $35. $15 for the nonrefundable fee, and then 20 for membership fees. I really don't know if I did everything right. Just woundering. Secondly, I'm going to get my Hep A & B shot today. I'm not sure if those 2 go together. Thirdly, where did y'all get your immunization? Do I need to print out the form so it can be signed by the PC? Thank y' all.......you're awesome!
