You need to ask yourself, is this an "untoward incident". If it is then you have a duty to document and report. If you don't then it could happen again, is there a problem with the handover process and how has this happened. Reporting procedures are about improving the care for your patients.
If you feel that this is acceptable practice then change the dressing and document.
The fact that you are asking, I suspect means you already know what you should be doing
Thanx guys. I reported it to the Supervisor who promptly blew me off and then tartly asked when I was scheduled to work again. I feel a cancellation coming on. Right is right hough and no one gets away with anything. It takes two minutes to clean a site with some NS and cover with a split 4x4 with adhesive edges built in no less. Geesh.

G-tube dressings should be changed, at a minumum, every 24 hours. Frequently they are changed 2-3 x per 24 hours. The opening collects secretions which if left will cause the area to become red and excoriated.
Personally I would fill out an incident report. If this was changed on the 3rd and today is the 12th, that is not good, not good at all. What would you do if that happened to your loved one? Report it? Do the same for the patient. Fill out an incident report. Patients are the number one concern.
**Sorry I didn't read all the posts first. So glad you reported it. Sorry the super took it so poorly. I'd still fill out an incident report, then they would have to deal with it.
