For me it depends on which scrub top I'm wearing & which assignment I'm on. If I get a run with a lot of combative people, I suck it up and put a lightweight long-sleeved tee on under my scrubs so I don't get scratched as easily, even though I sweat like crazy all day. If I'm on dayshift with my grabby (female) coworker I wear a cami & tuck it into my pants so she cannot grab my skin in the muffin top region if my shirt rides up while I'm working. Yes, I've asked her to stop but she won't so I try to keep myself covered as much as possible. And lastly, if a top fits in a way that might show any cleavage, I wear one of my husband's A-line undershirts ("wife beater") under it.
Rare are the blissful days when I can wear just one shirt and be cool and comfortable. Maybe I should go do some shopping next payday & try to find some tops that cover all my bases.

Fact is i sweat ALOT at work. And its nonstop. I start beading up not long after I walk in the door and im still dripping 12 hours later when im scheduled to get out of there. My LTC center uses polyester polo's for company shirts and they are 1) fairly see thru and 2) hot as hell. I've been wearing a plain cotton Tshirt under it but wondering if anyone else has this problem and has a better solution.
And those that wear scrubs, same question to you. Have you found a type of shirt that works best as an undershirt?
