correct dosage times for various meds

Ok is there a ROT (Rule Of Thumb) for the time in which.certain meds are to be given?..like obvious if.a order indicted a med to be given qid we know thats 4x a day within 24 hr so every 6hrs ...but some times to divide it equally is not a reasonable time for pt to take I.e. 0000....so is it ok to cut a few hrs here and there for when they are more likely awake?..and also I've learned that with ABTs its best bid time is like 8am and 8 pm but what about tiD and qid ABT orders....

I make an agreement with the other shift when this happens and there is no written policy. When given the chance to schedule meds, best to move the time to 15 minutes before or after. I will give the med at the ten minute before mark to help out the oncoming shift. Of course I tell them, and chart, that the med has been given.

When I worked in the hospital, that would fall to you, the offgoing nurse. Give it at 2:45.

Find out what your policy is for change of shift Medications. Where I work the off going shift is responsible , they've had the patient for 8 hours and know if there are contraindications to giving a dose to that patient. The oncoming nurse is getting report and may not finish handoff before the medication could be considered late. However when able its best to avoid medications at those hours as they aren't our standard medication times.
