The DON at my snf requires us to click off all the TAR the wound care nurse doesnt. I believe this is falsifying medical records and am not sure how to address this with my employer.

Assuming your post is means that the wound care nurse (supposedly) does the treatments and you (not the wound care nurse) are responsible to mark these things as done on the TAR:
Say no. Literally.
Unless this is some weirdly nuanced situation, this is one of those things: The person who performs the care needs to sign it off, for both legal and ethical reasons. Unless you want to click on every treatment on the TAR and write "Reported as being done at xyz date/time by Betty Boop, RN" (which is completely stupid) then just say no. "I will not sign off unwitnessed cared that was supposedly done by another licensed individual." End of story.
The wound care nurse is not performing the care. I don't have time to perform the treatments whe also providing med pass and other tasks for 21 patients.

I had a similar situation where I executed a 5250 hold that had not been done and was two dfays late. By this I mean that I timed and dated the document in real time. The supervisor qanted me to change the date back to the original due date and I refused. After some back and forth I stated to her, "Just to be clear you are asking me to commit fraud and falsify a medical record." She stopped talking and walked away."

If it had been done, I would consider writing a note, similar to what @JKL33 wrote in their post above, that the would care nurse reported having done it. However, since it hasn't been done, there is no way that I would sign of the entry on the TAR that it had been done. And, you are correct that if you document that you did it that would, at a minimum be falsifying the record, and likely fraud if the facility billed for the treatment.
The bigger issue, in my opinion, is why the wound care nurse isn't doing the treatments.
Best wishes.

This is reason 178 of why I hate nursing homes. It is absolutely horrible when your boss, who is a NURSE, asks you to document that you did something you didn't do, and don't have time to do. I always refuse these types of requests. I have never been written up for it, never been fired for it, but I usually quit because it makes me feel miserable and paranoid at work. So sorry you're in this position, and I hope things get better.
Thanks for your response and kind words.

I would not do it! It is falsifying the record and by you "clicking" it on the TAR you are documenting that you personally performed the dressing change. Tell you DON that you cannot do that but occasionally (key word occasionally) if another nurse forgot to document that you will put in a note as described above (that so and so states they did it). Your DON knows that the nurse who actually performed any of the treatments needs to document it!! Don't put yourself at risk!
