Central Line Dessing Change

Hi, I'm a first semester RN student and I have to document a central line dressing change and am having difficulties figuring out how I should do it. Could anyone give me an example please? Thank you

If I'm understanding correctly, you want to know what information needs to be entered into the patient's record? Since every organization has a procedure for actually doing this (and other) clinical procedures, you don't have to describe all the details of the task. For a narrative entry, something like "R subclavian dressing changed per procedure; site intact without S&S of inflammation".

If you're dealing with an EHR system, it will probably have some checkboxes to accomplish the same thing. Of course, if you DID notice any signs of inflammation or infection, you'd have to chart those & take appropriate action, including notifying the physician.
