Cali to Texas endorsement verification issue

My original license is from New York, where I currently practice. However I had my license endorsed to California and now I am relocating to Texas. I accessed nursys to verify my new York license, but now I have to use a verification paper form to verify California. Is that the single page form provided by Texas BON website? The 3rd part of that form is the nursing program/exam asking info about my nclex score and school graduation date which was all done in new York. Who fills out this part? Me? Cali BON- if so, how do they acquire my info. Is there another page I fill out for verification. The verification fee is also now $100, which is ridiculous.

Lastly, for fingerprints, I just want to confirm that I have to first make an appt. with Morpho Trust to do this. Do they hand out fingerprint cards or do I have get them from the police/court house or whever I do my fingerprints at. Who do I send the cards after?

Do I have to send any transcripts or photos to Texas.. because CALI-BON required that.

I think that, in your shoes, I would want 100% accurate information on how to proceed. That really can only come from the state Board of Nursing. Would you really trust an online opinion for such an important question?
