Just to make ABSOLUTELY 110% SURE--I've done all of my CEU's on CEUFAST, and these are the courses and certificates I need to turn in:
Infection control and barrier precautions 4 hours
AIDS/HIV 4 hours
Domestic Violence 2 hours
Tuberculosis 2 hours
Hepatitis C 2 hours
Medical Errors 2 hours

Going to dr office tomorrow. Coming out WITH titer results. Have my tetorifice/diptheria immunization and PPD. That should be all I need. Then would just have to bring it back to that dr who did the TD shot and get him to stamp it and sign it or whatever.
I'll be sooooooo happy to have this all done!!
Well, my husband woke up bright and early and got our results-problem is (one of a few, of course), they are COPIES. They said something like they mailed us the original physical forms. ***** We told them explicitly we need the originals. I swear they are just doing everything the opposite of what we say we need...
Also, my sheet where the dr needs to fill in date I got titers done- aren't filled in by him, although it is signed by him at the bottom. Sure, I could fill it it myself. Minus it's a copy. (And we need the original.)
Also, my pediatrician records state I got 3 Hepatitis B vaccinations. I got the Hepatitis B titer, and it shows I'm not immune to Hepatitis B. So....guess that takes precedence......RATHER unfortunate.

My Hepatitis B came back as if I had never had taken it before too. I received the series in the year 2000. I just filled in the dates when I received them. The clerk @ the nursing office said that if anything is wrong, the administrative person that goes over a final check off, will send an email. If u do not get an email, then everything is fine.
Also, I am little bit dumbfounded about the dotctor mailing ur physical forms instead of giving them to u. Sometimes, I believe, they just do what they want to do. U need the ORIGINALS to give to the school, all the other documents will be copies (this is the folder they will keep). But u however, need to make a copy of the physical to keep with all the other original documents (this will be ur folder to keep in case they lose anything). But still bring both to the campus when u go so they will see that u have both folders, even though they will give u the one back for u to keep. Also, just fill in the dates for the titers, mine were not filled out by the doctor, the medical assistant filled it out. The doctor just did the physical and signed off on it.

I feel like they are truly THIS close to getting my husband and I unable to start Nursing in January.

It's ok, let the quest results stay as is. Just fill in the hepatitis B dates on the page that asks, if u have taken the vaccine, fill in the dates there. U have only 2 days left, yikes! I would ride their butts like coattails. This is very important for both u and ur husband.

My husband walked into the dr office this morning with our son (thinking our son might fuss, leading them to want to give us what we need to get him out of there quickly), and he left with copies of our physicals/lab results, the office claiming they had already mailed off the originals to us.
One of my pages, with the negative PPD results, was missing the dr signature (how very professional, but we all already know just how professional my doctors office is, right?). Anyway, some other miscellaneous things were missing or just didn't look right on my papers. AND the results from Quest, the titer results, showed that I wasn't immune to Hep B.....Which confuses the HECK out of me!!!!!!!! I have my pediatrician immunization paper which shows my three Hep B immunizations. ??? But since I did the Hep B titer (my husband didn't-he already knew he had his Hep B immunizations as a kid and had his pediatrician record when we did titers, I didn't till the day after...or else I wouldn't have done that titer, and wouldn't have this issue now), that supercedes my old immunizations. So I am going to the Dept of Health tomorrow and getting the first series of shots done. Sigh. I'm also attaching my pediatrician immunization records and writing the three records of shots in, of course, so they know I'm doing EVERYTHING I can to cover my ass, lol.

You turn things in at bldg 8. They check it and notarize it. A nurse will look through the physical forms for any issues and if she finds any she will email you. I would call them up because as I understood it, you had to drop things off by the 1st of December, although I may be wrong.....I wish you the best of luck. I just got my stuff in today and had been stressing since orientation to get things done. Believe me you guys aren't the only last minute-ers.

Yes, that tetorifice shot sure does hurt...I got it done like 3 weeks ago and I can still feel a little soreness.....oh well. I hope that's not one of the ones we have to get every year...
