Hello!!! Just wondering if anyone else has applied for August 2010 yet? :)

What is the difference between the form SHE sent us in our email and the form we print here ?

Do we do both of these pages or just the one SHE sent us because these forms looked the same except for one being notorized

I did my fingerprints today, it was fun :p Even though it only lasted for 5 minutes. They do it electronically now!! The movies need to update their technology

I suggest you submit the entire form that SHE sent in our email. All of the pages are slightly different.

I submitted the old form with the new notary page and it was fine. As long as you printed the form that contained the page that you are not restricted from participating in clinicals you are good. :)

I always follow direction. The pages are different and if you have the correct pages that SHE sent then why wouldn't you use them. Better to be safe than sorry when it comes to BC.:D

I had my physical three weeks ago, so I used the old form. I already handed in all of my paperwork and the nurse reviewed it with no issues. If someone has not yet done their physical then of course they should use the new form, but if it is already filled out there is no need to have it re-filled out...that's all I'm saying. :) Good luck to everyone!

Hey all! RE: the physical form. I reviewed each side by side. The only pages that are slightly different are the first informational pages (which are for you) that explain the vaccinations needed. Then that last page that needs to be notarized. So the original form off of the website is fine to turn in, you just need to have the new notary page that was emailed with it. Also, where do we turn in the paperwork? Is it specific to the campus you are attending? And if so, where would that be at South Campus? Thanks!

that is what im trying to figure out. i think we will have to wait for the email that gives us instruction on what to do with our paperwork. remember at orientation she said each campus is different and has a specific way they want the paperwork put together (binder/folder, etc.). and i also believe it has to be submitted at the campus we are attending. they just forgot to print the instructions for south campus to attatch to our schedules [yay :rolleyes:]. so we will just have to wait for that email. hopefully it comes soon. im getting all of my stuff done this week!

Me too! I actually found a place that was reasonable for the vaccinations I needed so I was happy about that, but this is all costing me a fortune!

Last year there was a little paper inside our packet that was specifically for South campus students as you mentioned. It said where to go and what time to drop it off. Make sure you make a copy of EVERYTHING. Just make a duplicate of your binder (except for those originals that they told you to keep) and keep it handy. At the beginning of every clinical you will be asked to show your copy of your binder. They also randomly select like 5-6 students to submit their binder to the dean for review so pleeeeeease don't forget it on your lab day. Luckily for you guys this semester starts with a new acting dean for south. SHE IS AWESOME AND THE BEST TEACHER I'VE EVER HAD.

Again (For South Campus):

1. DON'T BE LATE= DO. NOT. BE. LATE.

2. BRING YOUR BINDER

3. HAVE YOUR NEW STUDENT ORIENTATION PACKET FOR LAB AND THEORY ON THE FIRST DAY.

i just got the email for south campus info! included in the email is an orientation welcome letter, booklist, packet checklist, and physical form which we already received.. And orientation is on the 2nd day of class [weird?]

I went to Quick Family Emergency care in Davie. Phone number is 954.640.1200. I paid $50 for a vericella titer, $25 for the ppd test, and $35 for the tetorifice shot.

My status finally says pre-admission!!!!
