I use dictation software at work because I'm a lousy typist. As a matter of fact, I'm using it to write this post. So today I'm doing a medical record review and I'm getting a lot of very entertaining choices from my dictation software. These all happen to be drug names, and it's funny because the software frequently spells the generic name perfectly well, but then the trade name gets mangled. Sometimes is the other way around. Anyway, here you go.
Lexapro (SQ Trollope RAM) {(escitaprolam)}
nor per mean ((the zipper mean)) {Norpramine (desipramine)}
Luvox (flu blocks me) {(fluvoxamine)}

I recall one time reading an MRI report where under indication for exam/clinical data said "the member..." when it was supposed to say "the pain is..."

Someone I know refers to our dictation software's quirks as "Freudian".

I often use Dictation on my iPad and Macbook. I wish there was a way to put commonly used words in a dictionary so that it wouldn't auto-correct to something completely different than what you intended.
I use Dragon Dictate for Mac (there is a PC version too). The newest upgrade is really pretty good, and it is easy to train it to recognize a new word, name, or just a regional pronunciation. I often put it in my plans for people with keyboarding difficulties (paresis/paralysis/spasticity) for that reason-- even pretty good for people with dysarthria if it isn't really bad.

I gave up on dragon a long time ago. I type all of my notes. It takes longer the first time, but I have few or no corrections at the end of the month. I am an excellent typist. I type about 80-90 wpm without error. When I was using dragon, there were so d@mn many mistakes, I was spending an entire Saturday doing chart corrections at the end of every month after medical records did their audits. Not worth it. IMNSHO, dragon just sucks. You can train that beast all you like, it still sucks. I spent untold hours reading it the damn lexicon of the English language and it never got appreciably better.
I won't use templates either, I just don't believe in them. I spend more time yes, but when we need prior authorizations, my notes are always sufficient and I never need to do extra work for justification like my colleagues who use templates. Anyone who reads my notes knows exactly what is going on. Not so with the templates. I am also confident I will never have to tap dance in front of a lawyer or a jury. I am a purist.
