If we were to implement something 'new' in our NICU, it would still likely be 10 years "old" by the time we got it going.

2-3 years ago we got much more aggressive about treating hypoglycemia. Until then, the reportable glucose was below 35, suddenly it became 50. Apparently the 35 number was not research based and could not be supported.

Just in the last couple of weeks, the MD's let us know they would start giving a much lower dose of intralipids. Previously, most infants were on 3g/kg/day. The concern is related to it's relation to pro-inflammatory properties that lead to liver disease and cholestasis. The newest guideline is to give between 1-2.5g/kg/day. At the same time, and I"m not exactly sure if these are related, we no longer turn off lipids unless the triglyceride level is greater than 300 (previously concern level was >200).
