I got an email from a recruiter for care manager positions with Senior Bridge in my area. I've looked at their website & I think it's something I may be interested in. It's not home health care per se, it is more interviewing and documentation. The point is to prevent hospital re-admissions.
Just wondering if anyone can share personal experience with this kind of thing. Thanks!
I've never gotten a "list to choose from". I get assignments, and have been told that they don't know exactly "what area they might be in, as they only get a zip code". I'm gathering that this isn't entirely true. I'm learning much through these posts, I wish I'd found them prior to starting the job as a subcontractor to a subcontractor to Seniorbridge. It's kind of disheartening to see what I could be earning.

Scroll ALL the way to the bottom of the page in the 4th column under the heading that say " locations. Click on the last one under all the states that says " care management network." That's what you are wanting. Read about the national care management network. There are phone numbers there. Always say you want to be a network care manager for humana/ seniorbridge.

One more thing.. You aren't considered a care/case manager with life plans or care scout. You are just contracted to do long term care assessments. You will see the person once.. Do your assessment and not see them again. With Humana/ senior bridge.. You are considered a care/case manager because you are over their case and following them either in the transitions or long term in home program. Hope this helps you better understand.
