Health Office 'Think Sheet'

Hello again! I am wondering if anyone has a 'Think Sheet' they use for students who are frequent visitors to the Health Office. I work with older students and have a handful of students who come to see me quite often; I suggested (oops, I put the cart before the horse) to a teacher that I develop a 'Think Sheet' for certain students to fill out to reflect on how bad they are feeling, what interventions that have tried, what might help if they went to the nurse...

Has anyone tried anything like this before? If so, do you have some sort of template? What are your thoughts in general about a process like this?

I want to make sure you know that there are some (first) names on the form as well as a phone number.

I feel like I talk out when to come to the nurse with my frequent fliers a lot, but the reason they are coming usually is behavioral/avoidance. I would be nervous that a think sheet would give them the road map to the nurse office instead of causing them to reflect on what they are feeling.

Thank you all for your input and for the example. I would like to incorporate a social/emotional scale as well.

My hope with this extra step would be to help the students learn to be more responsible, more resilient, and (probably most importantly) to create an extra step that they might be unwilling to take before coming to the health office :)

I like this idea for the older kids--maybe 3rd and up?

I don't have anything like this but have initiated in previous years a once-a-week or even once-a-day nurse pass, and once it's used that is it. It does still guarantee that I see that particular student, but if they use their once a week pass on Monday, well...they think harder the next week. It does not introduce the thought process like you have presented, like maybe helping them learn some self-care, but it does make them think it through before leaving the classroom.

I may combine the two concepts for a few of my frequents. Just the ones that never actually have something wrong.
