second trimester abortion of fetal demise, ever do it this way

we were just reviewing some of our policies and was wondering if anyone has done or is doing 2nd trimester abortions of fetal demises with pitocin as follows:

mix 100units pitocin in 1000cc of LR and run at a rate of 100 to 125 an hour until aborted?

i have never done this and actually never heard of it. help me out:)

The day before: placing 2-3 laminaria in early am...then removing those and placing as many as possible (usually 4-8) makes induction the next day much faster and easier on the patient.

We have a couple of docs that do it this way on some pt's. Must be no farther than about 25 weeks, no previous hysterotomy (obviously including prev. c section.) I think that it works well. Naturally I make sure these pt's get very liberal pain control (as with all demises.)

Had one doc suggest it, but unit manager overruled. Doc said he had done this at previous hospital he was at. Unit manager said she had heard of it, but there was no unit proceedure or policy on it so we couldn't do it until there was policy and proceedure in place. Only time I have every heard of it.

have seen this used w/ elective (really gross topic) ab's in 2nd trimester. 1st pt had laminaria about 24 hrs. prior, then urea injected into amniotic fluid on day of ab, then hemabate inj. into the uterus to start uc's. after water broke, 100 u of pit. in a 100cc bag of lr at 125 cc/hr. lots of retained placentas. we use vag cytotec where i work now and only for demises in 2nd and 3rd trimester.
