    You will need to fill the instruction sheet(examination part ) attach criminal back ground from which you will get from your city police station , then send this to board of nursing , and mail your finger print card to ISP or any live scan vender , they will send your finger print result to board of nursing if everything ok with your form and finger prints then they will send you ATT by mail . And your finger print results are valid for 3 years .
    Thanks a lot. I have sent my application, finger print and fees. I have question though; I haven't found any form for SSN. I want to tell them that I don't have one. All I found is this like regarding SSN for international nurses http://www.idfpr.com/Renewals/apply/FORMS/CGFNS_02.pdf

    If you do not have a US SSN then just submit a letter with application or leave blank on the forum. Unless in the US with a valid work/immigrant visa you can not apply for a US SSN

    Illinois board of nursing will allow you to sit for nclex without SSN , USA valid SSN only required during your license number renewal . They will give you temporary SSN number to appear for your exam . But when I applied for 2nd time via online they asked me to fill SSN with all 9s this only for those who does not have their SSN number . I appeared my exam with out SSN number when I got my result in that from I saw last 4digit of temporary SSN number.

    I remember receiving a letter from the BON that they have received my application and they need the fingerprints. That's when I went to Chicago to do mine. Because I know the fingerprints are only available for a period of time.
    How about if I plan to endorse my nevada RN license to illinois? I am also a philippine grad.

    Then follow Illinois' instructions in licensure via endorsement. Heard it takes a while so start soon.

    Hi everybody, CGFNS just received all my requirements for the CES 2 weeks ago. How long should I wait before they issue the report to ILDPR? Coz I want to do this online review which expires after 90 days, I want to make sure by the time this review expires I can sit for my NCLEX...considering I don't have any problems with my transcripts.

    And when should I submit my application to CTS?

    You can always start studying now. I know the permit is valid for 90 days. So, you'd be tight on schedule if you study later.

    Wait for the CGFNS to be done. There might be problems. Assemble your requirements/packet for CTS while waiting. So you can send it in once the CGFNS report is ready. Remember, the CGFNS report is available for 120 days.

    Hi! i hope you're still active here but i really need some help. Im currently living here in IL. I just came here last Nov. I am a Registered Nurse in the Philippines and planning to take the State Board. From the posts that I've read, I need a CES Professional Report which I am doing right now. I made an CGFNS connect account and while answering i becoming more and more confused. After sending the application online do i still need to mail my requirements? like my transcripts? and do i still need my highschool diploma? because i only have my college diploma with me. i hope you can help me. anyone please?

    You need to arrange for documents to be sent to CGFNS and generally documents come direct from establishment. Check the handbook out
