Kababayan, you just need to google or look at the optional states as listed below with the different states BON's. Google the state's BON for the complete application forms listed under "International applicants", print out the PDF forms and line them out as they are completed, it should take no more than 20-30 minutes to fill them out.

There are several states that are enforcing the concurrency rules (CA and NV especially) so that needs to be taken into consideration. But it really comes down to where your cousin wants to find and get a job the minute he/she passes the NCLEX-RN and forget about doing this endorsement process as it's a waste of time and money to re-do the steps over again. If that desired state requires the course evaluation AND English exam, do them, there's NO way to get around them.

Many states require a copy of local Phils nursing license so make sure he/she has one. Some states don't care now if one is a US or dual citizen and can't take the PNLE, they will not no longer accept the application until provided.

The old endorsement process of getting an out of state license then try to go back to a state with the harder restrictions (the "back door" approach) will not work as one must still pass that "new" state's educational requirements as if applying for the first time, even with many years of paid RN working experience. It will ALWAYS fall back to your original college school BSN transcripts.

You asked about the SS# requirement, so it's unclear whether or not your cousin is of what citizenship status, there are a few states that do not require them, BUT they will also NOT issue the license and license numbers till provided it's the SS# that passes the E-verify computer system, meaning it is a "work-authorized" SS#.

Even if somehow the state BON releases the license in error, any legit US employer will need a copy of the SS# so that they can again E-verify the numbers to show it's valid to work legally in the States.

Remember one does not apply for the NCLEX-RN but to a state board of nursing for licensing as a registered professional nurse by examination. Once credentials are evaluated authorization is granted to take the licensing exam. You register for and schedule the NCLEX-RN once approved by a state board of nursing. NCLEX is but one step in the licensing process.

It is a federal mandate that states require candidates for professional licensure ( not just nurses) prove they are eligible to work in the US by providing the state licensing authority a valid social security number (which is verified). This is slowly being implemented in all states. Some states like CA, MD, VT & AZ require the SSN as part of the application, no valid SSN = automatic rejection. Other states will not issue a permanent license/license number until the valid SSN is provided. There are some states still determining how to implement this mandate.
