hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

Hi may i ask how did you ans your declaration of nsg edu esp on no. 5? Do i have to include my CHN rotation? And immersion? Just to come up 1,600 total in hours workred?

Do we need to comply with the total of hours?? or just the hospitals we went on duty?

I will only place 4 hospitals.. they will only reach like 200 hours.. do u have a license in the Philippines?

nope.. not just in one hospital.. someone told me that I don't need to put everything.. I can put what I remember

How did you guys proceed with that?, especially the date started, date completed and the numbers of hours worked because i don't have a copy of my clinical rotations when i was studying and we had like many clinical rotations from different hospitals from 1st year to 4th year. I'am planning to contact BVNPT tomorrow but still i want to hear from you guys on how you completed your forms.

i have called them about q#5 and was so concerned about it. i remember about the other members post here and what the lady isaid when i called bvnpt. to just put what you remember in terms of school and dates

I proceeded with that, planning to submit my declaration form tomorrow, how long will it take to get my eligibility after i submit my declaration form?, does it take months?,

Mine took a good 2 weeks. I was left in awe how fast the response was. I really wasn't expecting it. :-) good luck on your application.

Request for another documents and if your school doesn't have a printed address,they should put a seal outside the envelope,then send it back.That's all they're asking.

Do you have to do the whole processing all over again when you fail the nclex pn exam?

Of course. It will happen. I hope if I do fail I don't need to do the process all over again. Ugh... ?
