1- oxygen

2- iv fluid - pt need need to hydrated first before oxygen because fluid is needed to reverse the clumping or agglutination of the sickle cells

3- rbc transfusion

4- pain medication

what will be the initial action if a pt with femur fracture caused by an accident,

1- take blood pressure

2- assess airway

3- assess vital sign- correct because the pt is not in a life threatening situation and the vital sign umbrella other option

im sorry but i have to disagree with this. a patient with sickle cell anemia, needs to have pain managment first. hydradtion may diluted the cells, but #1 is pain. second, a pt with a long bone fracture needs to have the airway assesed due to the release of a possible fat embolisim going streight to the lungs. i love this thread. i am taking notes. i am on post 129.

i can see how student don't read question before making decision because they are reading too much into question.

a pt in crisis should be hydrated first because the lack of fluid will coagulate the blood which will lead to pain and if the pt is hydrated it will hepl more than pain medication and the question is asking for priority question which make hydration the best option follow by the rest such as oxygen, pain rest etc.

2nd question does not specify if the pt have fat embolism but the question is asking you, what will you do for a pt who have an accident, what will you do and the pt is not in any threatening life situation which make assess vital sign correct.
