I encounter this too! In psych institution at saunder's Q & A. The client told the 2nd shift nurse " The first shift nurse allowed me to smoke 2 cigarette awhile ago. "..And the correct response by the second shift nurse is You're only allowed for 1 cigarette. And this is not my answer. I answered you're not allowed to smoke I got confused with this question too. It's in psychiatric area.
also had the same question in kaplan-- i picked the one where i don't allow the patient to smoke-- but i was wrong-- the answer was you allow the patient to smoke after the bath -- kind of like an incentive..?! hehe
i think the patient was a psyche patient and that made it ok for him/her to smoke-- and the goal was to let the patient take the bath--

I just came across a question where basically the pt wanted to go out to smoke but was in the middle of a bath and the correct answer was to state to the patient that they could have a cigarrete after the bath. Is it because they are in a psych unit? Because here, our patients aren't allowed to go out to smoke so naturally I didn't pick that answer:/
