Med/surg patient worksheet

Does anyone have a good shift worksheet for at least 6 med/surg patients? I'd like to be able to carry one simple worksheet throughout the day and then use it during shift report. I am not too computer savvy, and I would really appreciate some examples. Thanks!!

I have a sheet that I absolutely LOVE, only for 5 patients, though...

I have a really neat sheet that holds 8 pts. It can be folded into a little booklet that can be carried in your pocket. Takes some getting used to but I can't work without it now!
