This test questions threw me for a loop... Please Help!

I just finished my 2nd med surg 2 exam and thankfully I passed ! :yeah: However, I did not like some of the questions, so I thought I'd post them here and you guys can either help me understand what I did wrong or agree that these are some really bad test questions.

Lecture- Vascular System

1) The client is 2 days post-op after having varicose vein surgery. You decide more pt. teaching is needed whent the pt. states I should not:

a. wear elastic compression stockings continously

b. walk 30 minutes/ day

c. dry my incision after taking a shower

d. apply lotion.

( I picked A and got it wrong)

2) A clients family history reveals lower extremity circulation problems. The nurse should tell the client:

a. Take your antihypertensives as prescribed

b. Do not consume more than 2-3 alcoholic drinks daily

c. Jog for 30 minutes 3x a week

d. Take 2 adult aspirins a day.

( I chose c and got it wrong)

3) A patients spouse has parkinsons disease. The nurse would expect which statement from the clients spouse:

a. He writes big with big loop circles

b. He has tremors in both hands when active.

c. He lads behind me and doesnt' know where he is when we go on walks

d. He can't hear me over the telephone.

( I chose c... the three cardinal signs of parkinsons is : tremors ( unilateral ), bradykinesia ( slow, shuffling gait), and ridgity.. C described bradykinsia but I still got it wrong...)

4) A client just suffered a stroke. The nurse is now determining whether the client can eat or not. The nurse deteremined that the pt. has an intake gag reflex, what is the next nursing action?

a. Assess if the patient can move head

b. Offer liquids so that it'll be easier for the pt. to follow

c. insert an ng tube

d. Sit the pt. in an upright postion with the neck flexed forward.

( I chose A and got it wrong)

These are the main questions that threw me for a loop. I don't understand why I got these wrong but I did. Before I reveal the correct answers I want to see what you all think first. And then when I got some of these questions wrong the instructor says " How could you have gotten that one wrong" I reply " I don't know, I guess I'm just an idiot ! "

I'm not in the program or anything so I'm just going with what ever comes to mind...but would applying lotion be bad for an incision?

so, #1 i'm not entirely sure. i probably would have picked the same answer you picked just because of the word continuously. maybe the answer is D? really not sure about this one.

#2, also not sure about this one. i'll just throw out B with no science to back that one up off the top of my head. just gut feeling?

#3, i'm going to say A. C doesn't seem right just because of the not knowing where he is part.

#4, i would say D to avoid any kind of aspiration whatsoever.

those are tough questions so i wouldn't feel bad about getting them wrong at all. most of mine are guesses anyway. let us know the correct answers! :)

#1. I would have picked D because you can't apply lotion to any surgical site. The question is kind of vague though. There are many types of surgery for varicose veins.

#2. I would have picked A because HTN is a major problem that leads to poor circulation

#3. B because it doesn't have to absolutely unilateral

#4. I was torn between B or D. because I have actually seen answer B in a practice test question. Actually used it in a presentation. D can also be right because sit them up and leaning them forward promotes swallowing. I actually probably would have picked B because this is a further assessment. You would usually give clear liquid to assess the swallowing more before offering anything else.

you must read the words in the stems of these questions very carefully. the answer choices are the patient states "i should not wear elastic compression stockings continuously", "i should not walk 30 minutes/ day", "i should not dry my incision after taking a shower" or "i should not apply lotion". when you read each statement completely as they were intended, answer choice a ("i should not wear elastic compression stockings continuously") is revealed to be a true statement and reflects that the patient was indeed listening to the teaching. compression stockings are never to be left on continuously and they need to be removed periodically. c ("i should not dry my incision after taking a shower") is true. no vigorous scrubbing of any kind of the incision should be done. lotion can be applied (d) if the doctor has approved it. the correct answer is b ("i should not walk 30 minutes/ day"). walking for 30 minutes a day will prevent the formation of blood clots in the leg.

poor circulation has to do with narrowing of the arteries and veins. peripheral resistance, resistance of the arteries against the flow of blood through them, is one of the components of high blood pressure and a cause of lower extremity circulation problems. the patient should take their blood pressure medication to prevent future problems.

every parkinson's patient i ever cared for had an intention tremor in both hands. it was the pill rolling tremor that was unilateral. c is wrong because while parkinson's patients are slow they do not get disoriented and lose track of where they are. d is wrong since parkinson's does not cause loss of hearing. as for a, they have an intention tremor and some spasming, so they would not be making big loop circles when they write.

I start in the RN program in the fall. But I do have some healthcare experience.

ok. the last two threw me off as the way it was worded. I thought the shaking hands had to be too obvious. I was looking for the trick.

So when it says the pt should Not do it, means they are repeating the teaching back to you?

These are the main questions that threw me for a loop. I don't understand why I got these wrong but I did. Before I reveal the correct answers I want to see what you all think first. 1.B is good, but it could also be D (you'd never put lotion on an incision, but putting lotion (via massaging or rubbing)anywhere on the leg could turn a thrombus into an embolism.

2.A (B, C, and D are not independent nursing actions- you can't just tell pts to do certain things- i.e. telling a pt to take ASA would be a doc's job, jogging could be contraindicated in this pt, nobody should drink three drinks daily- sounds like you're giving the pt permission to drink two drinks- besides alcohol is not directly related to circulation). By telling the pt to take his anti HTNs as presribed, she's reinforcing and encouraging compliance with ordered tx.

3.B(not sure, though)

4.D (Next logical step- to prevent aspiration)

1. C (Rationale: u should dry the incision site p showering) Answer A is a correct statement, so more teaching would not be needed.

2. A On this question, I would've had to use the process of elimination to figure out the correct answer. I would not pick B because the correct amt. of ETOH to recommend on a daily basis is no more than 1-2 drinks, depending on your gender. (Male is 2 drinks and female is 1 drink per day) I would not have picked C because 30 mins. 7X/week is the correct recommendation for pts. And I know that aspirin enhances circulation, but the dosage is high. ( I thought you are supposed to take baby aspirin for this purpose as opposed to adult aspirin) So that leaves answer A. This would be a correct recommendation for any pt. that is prescribed antiHTN meds, even though the stem does not give alot of info about the pt.

3. B Unilateral tremors is at the beginning stage of the disease. Does not give alot of info in stem, so it could be as the disease progressed. I would have thought about picking C also because these are signs of the disease. ( forgetfullness in late stage, but don't think the pt. will be walking at this stage) unsure?

4. D Just because I've worked with this type of pt. b4

Just wanted to say thanks to Daytonanite sp?(I can't see your name now). I don't start nursing school until August but I always read these threads to see what I would have guessed, I would have got the Parkinson's one correct, probably because I just covered Parkinson's in my Patho Class in the past few weeks and it was on my Final last week.

The first one though I would have fumbled by the wording, but after you pointed it out and I read it that way it makes complete sense.

The last one I had no idea we haven't covered anything like that.

Question 2 I think it was, where the pt. has family hx I picked the Dr. one as well.

Anyway, having you go through them all like that was really informative and eye opening. I am going to have to for sure get help with test taking techniques. It's my biggest downfall. I know the next time I see NOT though in the question I will remember your post and read it in that manner.
