I wish I had a nickel for........

  1. I wish I had a nickel for every time a patient presents to the ER with an extensive list of medications, that of course doesn't have the list with them because we have it "on file". HELLO!!!!!!!

    They came up with a new system that ties in with our new tracking board. Now when we admit someone to the ER, if they have been in the hospital within a certain amount of times, the registration will spit out what they call a PAM report. Past medical history, allergies and medications. Sounds good at first, but when we have this report, we are supposed to take it in the patients room and go over the whole thing with the patient to see if everything on there is still current and then fax it to the pharmacy! AS IF!!!!!!

    We'll see how it works.

    We have EMSTAT in our hospital which does provide a list of past visits (for the past year). However, we are required to ask for meds/allergies again because things change. That's how I always phrase it: "Its important that we have up to date information and medications and allergies can change, so in order to give you the best care possible, could you please tell me what your current meds and allergies are?"

    Those programs sound really neat, but we are much too small to get something like that.

    Tried the "in order to take better care of you" approach, then you get, well, I have a blue pill for blood pressure and a pink pill for pain etc.....

    Gotta luv them, but their med list is with the doc in the clinic. We can pull up an old record, but unless they've been admitted recently or in the ER (with their list) recently it does us no good.

    Tried helping them make the list to have for next time, ooops, forgot to bring it.

    Usually we can figure it out one way or another, today sent a family member home to get the list because we had a patient that we NEEDED to know a list of meds.

    Just a nickel for my thoughts today (thought about all the times that happens and how rich I'd be :chuckle )

    :chuckle we have that too. if we have a patient that has been seen in the ER in the past month, we have a copy of the ED record on file. sometimes we get lists from there.



    here's a nother nickle thought......

    i would like to have a nickle for every time i've heard "is it going to be much longer before we see the doc? get labs back? get a room?" etc. ad nauseum.

    I'd like to have a nickel for every patient I get up on the floor who has pages of home medications, who brought the list with them, gave it to the people in the ER to copy, sent it home with family so it wouldn't get lost. Is the copy with the paperwork - NEVER!!

    I'd like a nickel for evey pt who has answered "none" when I ask them what surgeries they've had.

    Just had this happen a couple of weeks ago. I said to the pt "But what about your limb amputations and your colostomy?"

    "Pt answered "Oh yeah. I guess that was surgery."
