Who mixes pitocin and mag?

We just started using Pyxis and a recent survey we were told that nurses in LD should not be mixing pitocin, nor mag. We should use standard premixed bags of these meds.

What mixture of pitocin do you use? Is it standardized?

What strength of Mag sulfate is used as bolus and then maintanence doses?

Obviously, who mixes it in routine situations? What does your hospital consider "emergeny" criteria which allows the nurse to mix these?

We're really trying to find a way to comply with the new pharmacy standards the rules folks are pushing.

We mix our own pitocin but pharmacy mixes our mag. We mix 10u of pitocin per 1000cc bag of LR or D5LR depending on the order. Our Mag is mixed by pharmacy but it is 1 gram per 10cc's. Most of the time we give a 4 gram bolus over 20-30 minutes then 2 grams per hour. Of course it depends on the order but that is what we do most of the time. Our mag is kept in the fridge and has expiration dates on them, pharmacy comes to stock our acudose once a day, or we call them if we are out of something and they will come stat.

We also mix our own Pit, or Mag is premixed. We use 30 u of Pit in 500 cc of D5LR, we usually start our Pit on 2 (2mu/min) and go up by 2 every 30 mins for inductions. We normally use 4gm bolus of Mag w/ 2 gm/hr maintenance dose, depending on orders, but this is the usual.

Both Mag and Pit are bought premixed and stored in the pyxis. Pit is 30 in 500NS, Mag is 4gm in 100 for loading dose and 40 in 1000 for maintenance.

We stopped mixing our own pit right before joint commision last year.

Our mag has always been premixed by phamacy, and starting last year, so was our pit. We have 20 units in 1000cc NS or D5LR...and it too lives in our pyxis.

My guess is those getting ready for JCAHO in 2005/2006 will find their pit and mag being mixed in their pharmacies very soon after that.

We mix our Pitocin 10 units to 1000cc LR. Our Mag comes in pre-mixed bags from the manufacturer. We usually give a 4-6 gm loading dose over 30 minutes, then maintenance is 1-2 gms/hr.

Pharmacy mixes our pit and mag. For pit it's 20 units in 1000mL. We can mix our own in ermergency situations.

We mix both initially. Pharmacy sends up mixed bags thereafter, unless it is 5p-8a, when it's closed. (Small rural hospital.)

We had a JCAHO survey in June 2004 and the nurses mixing pit thing was OK at the time. Surprisingly.
