Don't ask... it's this stupid worksheet and we HAVE to come up with a nursing diagnosis... I can't find one that fits. The only thing "wrong" with him is that he doesn't eat breakfast and he knows it's unhealthy. None of the health/knowledge deficits have "related to" things that work.

How about this one:
Ineffective denial R/T feelings of ambivalence towards proper nutrition, as evidenced by refusal to eat breakfast.
It's kind of a weak scenario. Nothing that says that you can't tweak or stretch things a bit. I like to add my own details to fill things out a bit, that way I can steer the situation towards something with a greater possiblity:
Maybe "pt has stomach/gastric pain" or "Pt has just started a hunger strike" or Maybe "Pt just doen't like oatmeal" or "pt is an insomniac and snacks all night" (so he's full in the morning).
Just think about where you could go with your assessment!
Hey, at least it will be original and works those (creative) "critical thinking skills".

Probably the best to use would be Ineffective Health Maintenance R/T ineffective individual coping. It's also possible to use Noncompliance R/T motivation and/or health beliefs, or Ineffective Protection R/T inadequate nutrition, although this last one might be a bit of a stretch.
