What are your thoughts on AROM?

Reading about vasa previa, and having had 3 patients with chorio in the past month... I'm beginning to be really concerned with my 1 cm, posterior, high, 50% effaced pts having a first AROM attempt.

I've been looking at the research that says AROM is mostly beneficial in speeding up labor in a pt who is 4-5 cm dilated.

OB RN's, what are your thoughts on AROM?

AROM's are pretty common in my hospital. It seems like we have SROM's that start labor, OR we have labor where pt. comes to the hospital and then gets an AROM. Docs do it sooner than midwives. It is supposed to speed labor, which is popular with docs and moms both, but the evidence shows it's not by much. I wish providers would look at that evidence. I know it's harder on the moms, and I've heard it's harder on the baby.

Wish they'd leave it alone, or at least wait until good & active in labor... but our MD's do it ASAP if they are around.

Some say it can really pick labor. If it is done it should not be done until the baby is engaged.
