I would respond by saying "Mrs. James, I've told you three times to get up. I am now going to have to have navukaur come in here and use a technique for a therapeutic relationship in order to discuss with you what the goal is in this communication".
My first action would be to find out why she doesn't want to eat - is she not hungry? Is she feeling down? Is she grumpy? Does she want a shower before she eats? Things like that. I guess that would fall under "seeking clarification".
