I was wondering if anyone out there could post a link or email a copy of an organized spreadsheet that they use to chart down all their info for each patient on their shift. I have just started my clinical rotation in Med/Surg and have found that our hospital gives us a tiny census sheet which leaves making notes on each patient pretty cramped and disorganized. I have seen some worksheets posted that involve very thorough charting for each patient and i hope to devise a list for each patient so that i give a good thorough report---i have heard the difference in report between a poor one and a good one.
Does anyone have a worksheet that they would like to share on this forum or by sending through email?

Just a tip, for when you're using and preparing these worksheets: I had individual sheets for each patient, up to my last semester. Then I had instructors who insisted on ONE worksheet for all FOUR patients, so I definitely had to modify and streamline quite a bit! The thinking is that you don't have time (and, truly, I don't) to flip through several pages to find the patient data you need or which page to record on. I keep this page folded, brochure-like, in my pocket for easy access :)
I now have one I use that works pretty well for me; I'm attaching it so you can use it as you wish. The way I use it is I circle what applies, then fill in brief details next to each thing. I use the notes section for diets, flotrons, upcoming tests, whatever. If you can't figure out my abbreviations, ask :)

The AM Assessment is designed for one patient at a time, which I used for the first few semesters to make sure I didn't miss a thing. Made doing charting MUCH easier, as I just followed my assessment sheet :)
