PRC Requirements:

Examination Fee - 900 php

Metered Stamp

Window Envelope

Completed PRC Form or OR/DR Form

OR FORM

*2 pages(Five Major Cases, Five Minor Cases needed)

DR FORM

*3 pages(Five Handles, Five Assists, and Five Cord Dressings needed)

4 pieces passport size photograph

Transcript of Records with remarks "For Board Exam Purposes Only"

Community Tax Certificate or Cedula

hi! just ask your school about the format coz every school varies in format regarding the cases...i think the schools provide their students with the format...it should be the same for one school....God bless...requirements? it's also posted in your dean's office or your nursing bulletin board...coz my school did...all you do is comply all the requirements needed and go to prc...then, have your scrubs signed by all c.i's...dean...chief nurse...God bless

CAn I ask what school your from? And how come your school is not providing you with the copy of PRC form? Information on the PRC for varies from school to school. Your asking a copy from someone with different school. Did you had the same affiliation? Same info about school? Same charged nurse, unit manager, DON, Dean of NUrsing, CIs etc.

Now go ask your school a copy! Do it like a good student and please dont ask someone a copy who gone to different school as you are. Because it is the school's responsibility to provide you with the copy of PRC form. You can also ask the your seniors about this but NEVER from someone who gone to different school! That's very simple and easy to comprehend, isn't it?

Just a reminder to everyone especially those who are graduating this October 2008 and onwards, there are new set of requirements in terms of format in cases by the BON. Please visit the BON website for more details.

RE: new PRC forms

Rule 1:Logic dictates that these forms should be applied only to the in-coming Nursing students in Levels I and II only of Academic Year 2008-2009 onwards until their graduation and until new issuances are released by the Board of Nursing;

Rule 2:All those filing applications for this November 2008 Nurse Licensure Examinations and the succeeding NLEs "prior to the effectivity of these NEW FORMS" should all be accepted by the Central and Regional PRC Offices without any condition. If there are any noted discrepancies or any untoward observations, the Board of Nursing requires appropriate documentation and reporting from the respective PRC Offices and should be received by the Board of Nursing until after the last day of the NLE. Everything should be directed as official communications to the Board of Nursing;

Rule 3:As a matter of policy, graduates ARE NOT TO BE PENALIZED for any discrepancies and therefore all applications duly submitted "on time" MUST BE ACCEPTED. The Board of Nursing shall take necessary actions based on official reports received by the same at the Central Office within the prescribed period as set in Rule 2;

As a general rule the Board of Nursing subscribes to the principle of "loco parentis". The college and its administration directly involved in the care and supervision of students/graduates are and shall be responsible and accountable to the lawful authorities.

i remeber way back befor we apply for PRC our school provided us the format the details for the requirments. how could this happen to you, were did you put your cases? hmmmm:clown:
