requestion letter of recommendation..please help

Hey..Oops, thats supposed to "requesting"...my bad sorry. I need 3 letters of recommendation for UTMB. I wasnt going to apply there but I just now decided that I am going to. I only have til Jan 15 :uhoh21: but I think if I email my teachers today that I could pick them up on Friday. Anyway, my question is how do I ask for one? This is what I have:
Hi. Hope you had a good holiday. I am applying to UTMB for the fall and they request that 3 letters of recommendation be submitted. Would you please be a reference? The deadline is January 15. I will be happy to pick it up from your office if you don't mind. Thank you so much.
Does that sound okay? Please let me know if I need to change something, take something out and what not. Does the holiday part sound cheesy? Anyway, please respond if you get a chance, I would really really appreciate it. I need to email them today if I want to get it this week. Thanks yall!!

These are the changes I would make:

I would start out with : 1. Dear Dr..... , if he/she has no Phd use "Dear Professor.....", looks more appropriate and respectful than "Hi"

2. say who you are and remind the teacher which class you took, and when

3. Conclude with "Sincerely" ....your name

Use complete sentences and correct grammar even though it's e-mail

Assuming that you did well in the professor's class, you could also say:

Dear Professor XXXXX:

My name is XXXX and I took your XXXX course during the XXXX semester. I am in the process of applying to schools for admission and remembered how much I learned and retained from your teachings. I was wondering if you would be willing to write a recommendation based upon my performance in class. The deadline is Jan 15 however, and I know this may be short of notice but I would really appreciate your assistance in this matter so that I can go on to fufill my educational goals. Can you contact me to let me know if this is doable for you? Thank you for your time.

As you can see, the deadline, which is rapidly approaching was mentioned here to give the professor a heads up. Because even though a professor most of the time is more than willing to recommend a student, sometimes they are annoyed when given such short notice especially with their own busy lives. Other that, this should get you your recommendation. GOOD LUCK!!!

Tell the person what it is for. A good letter must have atleast 3 excellent qualities that you possess.

in addition to the reccommendations above i would include a notation of why you are asking on such short notice . they may assume that your first requests were turned down and they are way down on the list
