I found an order not transcribed by an RN. Nothing unusual for this person but while checking orders we are supposed to date initial and put if they are correct. So I did all the above as usual except I added that this order wasn't transcribed correctly and proceeded to make necessary corrections. Done this a million times and never had an issue.
The boss comes to me and tells me I was wrong for writing this, which mind you is what she told us to do a year ago. She said it's now stuck in the med record. That's also true but I don't think I'm the one entirely at fault when this RN she likes so much has made this same mistake countless times. That's not counting her many med errors, lack of any documentation, and lack of general nursing know how. She didn't know narcotics had to be counted among other things. She also didn't know if you get a new order from a dr it must be documented.
Yet nothing ever happens to her or other nurses who do worse than that. The boss came across like I was just trying to get the other one in trouble and that wasn't the case. Not at all. I'm just sick of having hours worth of corrections from other people. Am I just totally in the wrong here or what? I'm confused .

that is just crazy. the original transcriber should be responsible for this transcription error. i assumed you work night shift and do chart checks. that's what you are supposed to do. did you ask the boss what you were supposed to do?
you don't know how many times i see/correct transcription errors every time we do monthly recaps.

It is not unusual that the person who corrects errors and helps the facility stay out of trouble by doing so, is jumped on for their efforts. Watch your back with the boss. She is, for some reason, actively protecting her favorite lately, at your expense. You don't want these reprimands to turn into a termination for you. Consider whether or not you might want to do a mild search for greener pastures.
Yep-I'm a night shifter. I'm watching myself very carefully & keeping detailed notes for myself...just in case. They hire people and give little to no training and ones like myself often get stuck with the aftermath. What a mess.

It's weird to me that it's part of the chart to say whether it's been transcribed correctly or not. Most I Would ever put in a chart is 'order clarified' -i question your facility practices.
