Hello everyone,
I am starting this to share information and keep in touch just incase some of us hear back sooner than others. This is for those who plan to submit an application to the ADN program for Spring 2022. Any questions feel free to ask and hopefully this is helpful(:

For those who have already bought the packages for the school, I was wondering if you can send me the list of the immunizations needed? I'm trying to purchase the packages, but I can't get past the employee history portion because I have to find out the specific start/end dates from my job from three years ago. Also, the only available date for this month with my primary doctor is this Thursday, whereas RCC's health clinic's closest date is the 29th, and I would like to get them checked early since I know for some of them, they require a period of over two months.

1. TB Test (PPD/TST Skin test or Quantiferon TB Gold Test or chest x-ray if positive result)
2. Tdap shot
3. MMR - Proof of Immune IGG Antibody Titers (if negative you need 2 shots)
4. Varicella - Proof of Immune IGG Antibody Titers (if negative you need 2 shots)
5. Polio - Proof of 3 shots or Proof of Immune IGG Antibody Titers
6. Flu Vaccine
7. Hep B - Proof of Immune IGG Surface Antibody Titer (need 3 shot if you’re non immune)
It doesn’t say it but I’m sure you’ll need both COVID shots or negative COVID test when we go back on campus!
8. You also need a physical examination and a doctor to fill out a clearance form for both RCC and CSUSB which I am still waiting to receive the form for.

I think you’re able to upload the actual proof of immunization like the official documents onto the website or have the doctor check off/fill out on the form then upload it ?
