As most of you know, I have been recommending initial licensure in NY for quite some time. And for those of you that did not heed the warning, you now know why.
New York is three easy applications only, next you take the NCLEX exam, then you get a hard license and can endorse to almost any state afterwards. Because of the requirements of the Special Credentials Verification for New York, you do not have to do any of the running around to obtain your documents. CGFNS will obtain them for you....after you sign the release and send them the money.
NY also requires two short classes that can be done on-line, one on Child Abuse, and the other on Infection. Both are available for about $15 each and take only one hour or so.
NY BON application can be found at http://www.op.nyed.gov. You only need to submit "Form 1."
The Special Credentials Verification can be found at http://www.CGFNS.org, as well as the Visa Screen Application.
Once you have received documentation from NY that your credentials have been received and approved, submit payment to Pearson-Vue for the NCLEX exam. http://www.perasonvue.com. They are the ones that will actually send you the ATT letter, upon approval of the Board of Nursing. Be aware that this ATT (authorization to test) is only good for ninety (90) days. You will have onlt that amount of time to get the exam done in, otherwise you will lose your payment and have to start again.

I can see why people are always thanking you. !! You are so quick to reply with all of this wonderful advice and im in need of lots of it.. i do so appreciate it very much. I have a lot to accomplish because i have a partner who needs me to be with him. He has cancer and we dont know how much time he has on this earth. I dont know of any other way i can get to stay in the US for more than a few months so im going to take the time to get a job and hope we have some time left together. I know th job process isnt easy either, but i will give it a try. Do you think i could accomplish everything but the nclex and then do the nclex when i arrive? i have the hospital where my partner has been a patient, willing to offer me a job when i can work I just hope they mean it. You can do the whole process while you are in New Zealand, except for the NCLEX exam. Closest to you would be either Saipan, Hong Kong, or Seoul. Just depends on where you can get the best package for travel. :)
hi suzanne4! i just have a question regarding Form1
I have 2 degrees- Computer Science and recently I graduated from my BS Nursing course and I'm also an RN in my country....the thing is in the Postsecondary schools I can only list 3 schools and my problem is that I actually went to a total of 4 schools....
1 for my computer science course then
3 for my nursing course (i basically transferred twice)
what should I put in the list??
can I only put the 2 schools where I graduated from or should I exclude the school where I got my computer science course?

hello again! i have another question....in Form 1 # 20 it says there
'If you checked any of the Boxes from B-H, enter your alien registration number or control number issued by the Immigration Naturalization Service'
Since I checked letter H how do I acquire for the alien registration number? i tried to check out the http://www.uscis.gov unfortunately i couldn't find how to get an alien reg number

sorry if i'm bombarding lots of questions, pls bear with me :)
i'm very interested in applying for the NY BON so I've downloaded 3 forms -the Form1, Visascreen certificate and the credentials verification service
in the visascreen certificate #17 -regarding English Language Proficiency ....should I take and pass an english language proficiency examination( for example IELTS) before sending the visascreen cerficate forms or can I do that even after sending the application?
also in the credentials verification form #11 about Registration and licenses....since it was only recently that I passed our local Nursing board exam and my license will not yet be issued until august....should i wait until august before filing the credential verification forms?
should I send all three forms simultaneously or not necessarily?
Assuming I've sent all the forms and I'm waiting for the processing.....during this time can I take the NCLEX-RN examination or should I wait for an approval or some sort of a go signal from either NY BON or CGFNS before taking the NCLEX?

Hi, suzanne! I found your posts very interesting that's why I signed up....I have tons of questions and I believe you can help me. I just passed the recent licensure exam here in the Philippines and I'm now in the process of preparing my documents for the NY licensure, CGFNS CVS and NCLEX. I have downloaded the forms needed, and I want to ask you on how to fill out certain entries for all the forms:
FORM 1
1.In question #17, do I have to attach even my elementary and high school diplomas?
2.In # 20, I am still in the Philippines and doesn't have a US visa. What shall I check in the choices?
3.In # 23, I know I can send my certificates of the courses later after completing it, the problem is I tried to sign up with RN.COM but I think they only allow those who are presently in the US to sign up. Can you suggest any other sites where I can take online courses and sign up even if I'm here in the Philippines?
CGFNS CVS form
1.In # 7, I do not have any US social security number or new york state assigned no. yet, can I just leave it blank?
2.In # 10, I am asked to write an explanation regarding gaps in my educational history, I didn't finish my first course and shifted to B.S. Nursing, should I have the explanation handwritten? Should I also have it notarized?
3.I know that CGFNS will be obtaining the transcripts and validation of license from my school and the registration authority, but I doubt it if my school will be responsive to CGFNS, can I just personally ask my school to issue my full transcript of records to me signed and sealed by the registrar or dean and I'll be the one to send it to CGFNS directly just so to facilitate the processing of my documents?
VISASCREEN
1.Can I send my Visascreen Application together with my CGFNS CVS form?
2.In no #12, what is an H-1B, H-1C, 212®, TN, permanent visa?
3.Do I need to send another transcript of records and validation of registration for the Visascreen or the ones I'll be sending for the CGFNS CVS will suffice?
4.Can the results of the English Proficiency test be sent later?
5.Does the Visascreen application have a certain time limit to complete all documents like the CGFNS CVS in which the requirements should be sent within 90 days otherwise, you have to re-apply?
Lastly, How soon can the New York board of nursing make me eligible to register for NCLEX? And at this time how long does the processing of the licensure take? My sister who lives in Connecticut told me that right now, it's taking them a long while to process the present licensure applications, is it true?

You cannot take NCLEX examination until you have approval from NY BON to do so, and you won't get that until they get approval on your credentials from CGFNS that is responsible for the Special Credentials Verification. Submit the forms now to CGFNS and just put that the date for your licensure is August--, 2005. For NY, CGFNS must obtain the documents, you cannot ask the school or licensing body to submit them for you.
Only one set of documents is needed, they can be used for both the CVS and Visa Screen Certificate.
While you are waiting for NCLEX approval, take and pass your English exams.
Form 1:
You need your high school diploma.....everything that it asks for, they want.
You are in the process of applying for a permanent residency, and include a copy of your passport. That will meet their requirements.
Are you talking about the two required classes for NY?
CVS: No SSN, so leave the space blank.
The handwritten or typed note is just fine, no need to have it notarized.
As stated above, all requests for documents must be done by CGFNS and only them to meet requirements for NY.
Visa Screen: One set of document is fine, just fill out the applications for both. No time limit, this is the first that I have heard of a 90 day limit for NY. News to me, have never heard of that before. Just go haead and send the application in now, it will take about four months on average before you will get approval to sit for the NCLEX-RN exam.

Yes, as long as you submit the application and check off the box at the top of the page. As well as pay the fee.
