Question about a student reporting RN mistakes

The patient was technically not my "assignment". We were paired up with two students per patient and all we were doing was PA and VS. We haven't learned anything about doing armbands and we aren't allowed to do anything without an instructor present at the moment other than talk with the patient.

So why did you decide to do something about something you haven't learned without an instructor present? And technically, if you work in a clinical setting, all patient's are your patient. If you're gonna use the old, oh it wasn't my patient, and, oh I haven't learned yet, then don't poke your nose in it. If you're indeed concerned about patient welfare than own up to the fact that you used poor judgment (no big deal, you're a student, and if I was the nurse involved I'd explain to you why it wasn't done) by not having faith in your instructor (you had already told her), by not making a greater effort to locate your instructor (she can't be too far, because without her you cannot do anything), and by not consulting the attending nurse (bad form). Now if you can take this with a grain of salt and learn from it, I believe you'll do just fine.

FYI, the PCA could have assisted you with the armband and hat. Don't overlook them as a resource.
